Whether its diabetes, arthritis, heart disease, cancer or any other chronic disease, those suffering from chronic conditions face a number of common challenges. It could be fatigue, confusion on treatment, stress or other related symptoms; all additional complexities that come with an already-difficult health condition. That's why Mercy has partnered with the Heritage Area Agency on Aging to offer the Chronic Disease Self-Management Workshop.
Six weeks to take control again. For 2 ½ hours once a week, participants are trained by specially-trained peer leaders. The program is designed to help you or a caregiver learn a healthy way to live with chronic conditions.
Learn problem-solving and action-planning focused on solutions for tiredness, depression, frustration and managing symptoms. There is a one-time fee of $20 for the course book and relaxation CD.
