Guidelines For Medical Records Requests

All requests for copies of medical records must be received in writing. If the patient is a minor (less than 18 years of age), a parent or legal guardian must sign the authorization. Urgent patient care requests may be faxed to (319) 398-6848.

Print a copy of the Release of Health Information Authorization form (PDF format). This form must be completed, signed and mailed to the Health Information Management Department at the address above.
