Assessment. 


Chemical dependency assessments are available to anyone seeking a professional opinion regarding their alcohol or drug use. Family members are encouraged to participate in the assessment. All assessments are confidential and generally are completed in one hour. Following the assessment, if professional services are recommended and accepted, referral to an appropriate facility will be made. 

Evaluation. 


Chemical dependency evaluations are provided for a fee to anyone needing a professional opinion regarding their alcohol or drug use. An evaluation is an assessment with a formal written report. All evaluations are confidential with information disturbed only with written permission. Family members are encouraged to participate in the evaluation process. 

OWI Evaluation. 


Chemical dependency evaluations for court ordered OWI cases are provided for a fee. A formal interview is conducted during which several clinical scales are employed. Following the interview a clinical impression is formed which is then reviewed by the facility director. A written report is issued within fourteen days of the evaluation. 

Assessments, evaluations and OWI evaluations are available by calling the Sedlacek Treatment Center between 8:00 a.m. and 4:30 p.m. Monday through Friday.
