Chronic Disease Self-Management Workshop

Live active. Live healthy. Learn successful living with chronic disease.

Whether its diabetes, arthritis, heart disease, cancer or any other chronic disease, those suffering from chronic conditions face a number of common challenges. It could be fatigue, confusion on treatment, stress or other related symptoms; all additional complexities that come with an already-difficult health condition. That's why Mercy has partnered with Stanford University to offer the Chronic Disease Self-Management Workshop.

Six weeks to take control again. For 2½ hours once a week, participants are trained by specially-trained peer leaders. The program is designed to help you or a caregiver learn a healthy way to live with chronic conditions.

As a result of the class, participants will receive solutions for overcoming tiredness, sleeping issues, a sense of being alone, breathing difficulties, loss of energy, and many other issues associated with managing a chronic disease.

We want to empower you to get the most out of each day.


Topics include how to:

  • Manage your symptoms
  • Communicate effectively with your health care providers
  • Stop feeling alone and isolated
  • Reduce fatigue, stress, frustration and anxiety
  • Make your daily tasks easier
  • Incorporate relaxation and breathing exercises into daily life
  • Develop an action plan and set goals
  • Feel empowered to take back control of your condition and get the most out of each day

Each participant will receive a copy of the book "Living a Healthy Life with Chronic Conditions."

Who Should Attend

Who should consider the course? People with chronic long-term conditions such as:

  • Cancer
  • Depression
  • Arthritis
  • Lung and Breathing Disease
  • Heart disease and Stroke
  • Parkinson’s
  • Diabetes
  • Osteoporosis
  • Fibromyalgia
  • Or any other condition that requires self-management

It is also for a friend, caregiver, or relative who would benefit from the problem-solving skills related to managing a chronic condition. You can be younger, older - it doesn't matter. What's important is a desire to learn the important benefits of these skills.
