Admissions
Hallmar is an intermediate-level nursing facilitiy, meaning admission can be the patient's choice, a family choice or recommended by a physician.
The admission process into hallmar first begins with the completion of an application that provides our staff with information about the resident, their medical needs and requested accomodation type. After receiving the application, a meetng will be held between Hallmar staff and the prospective resident and/or legal representative to discuss admission and to ensure your family's goals and wishes are met.
Expenses
Costs of admission at Hallmar vary based upon the resident's level of care needed and their room accommodation. Hallmar is a private-pay facility, meaning that our services are not Medicare or Title XIX certified. If residents hold a long-term care or nursing home policy, Hallmar will provide the resident and/or their representative with the documentation needed so the family can work with the insurer to ensure reimbursement is collected.
We work with families in varying financial situations, and we want to work with you to ensure our residents receive the most cost-effective quality care.
Levels of Care
- Minimal (needs minimal assistance.)
- Moderate (requires some assistance dressing, toileting, etc.)
- Heavy (total care.)
