Mercy Cedar Rapids is hiring Home Care Registered Nurses to provide skilled, one-on-one care for patients in their homes.
Home care offers a different kind of nursing experience; one built around independence, relationships, critical thinking and meaningful patient connection. As a Home Care RN, you’ll manage a caseload of patients, coordinate care, educate families and help patients safely recover, manage chronic conditions or remain independent at home.
This role is a strong fit for nurses who enjoy autonomy, patient education and seeing the full picture of a patient’s health.
What You'll Do
- Manage a caseload of home care patients
- Assess, plan, coordinate and evaluate care based on each patient’s individual needs
- Provide skilled nursing care in the home setting
- Educate patients and families on medications, care plans, safety and recovery
- Communicate with patients, families, providers and care team members
- Coordinate services and resources to support patient needs
- Document nursing care accurately and in real time
- Respond to changes in condition or emergencies within scope of practice
- Promote patient safety throughout the home care experience
What We're Looking For
- Current RN license in Iowa or a compact state
- Current Basic Life Support (BLS) certification through the American Heart Association
- Iowa Dependent Adult Abuse and Iowa Child Abuse Mandatory Reporter training required
- Current valid driver’s license, valid auto insurance and reliable transportation required
- Bachelor of Science in Nursing preferred
- Previous RN experience in an acute care setting preferred
