Physician/Provider Candidates Qualifications/Licensure:
- M.D., D.O. or N.P./P.A. licensure or foreign equivalent
- Ability to be licensed without restriction or limitation in the State of Iowa
- Board certified or board eligible
- Registered or the ability to be registered with the Federal Drug Enforcement Agency to prescribe controlled substances without restriction or limitation
- Completion of residency or training program prior to commencement of employment
