Book your vaccine appointment for one of the following timeframes:
- 9 a.m. to noon - or - 12:30 to 3 p.m. on Tuesdays
- 7:30 a.m. to 12:30 p.m. on Thursdays
Note: Pediatric doses are not currently available; we will update this website when we have pediatric doses in stock.
MercyCare Monticello is available for individuals seeking a first or second dose of COVID-19 vaccine, as well as a third dose for those who are immunocompromised. At this time, we have Moderna, Pfizer and Johnson & Johnson vaccines. According to the CDC, high-risk individuals eligible for a third dose are those who have:
- Been receiving active cancer treatment for tumors or cancers of the blood
- Received an organ transplant and are taking medicine to suppress the immune system
- Received a stem cell transplant within the last two years or are taking medicine to suppress the immune system
- Moderate or severe primary immunodeficiency (such as DiGeorge symdrome or Wiskott-Aldrich syndrome)
- Advanced or untreated HIV infection
- Active treatment with high-dose corticosteroids or other drugs that may suppress your immune system
1. Confirm Your Eligibility
You are not eligible to receive the vaccine at this time if you have been treated with convalescent plasma or monoclonal antibodies for COVID-19 in the past 90 days.
You must be 5 years of age to receive the Pfizer vaccine and 18 years of age to receive the Moderna and Johnson & Johnson vaccines per the FDA. If under 18, a written parental consent (PDF) and consent to treat (PDF) will need to be completed.
2. Important Information About Your Appointment
- If you have already received a first or second dose, please bring your vaccine card to the appointment.
- Please bring an ID.
- Medicare patients: Please make sure you bring your red, white and blue Medicare card to the vaccination appointment. See an example here.
- While the vaccine is free, your insurance may be billed for the administration of the vaccine.
- Please plan to remain at the clinic for at least an additional 15 minutes after your appointment time.
Scheduling:
Please indicate in the “Reason for Visit” field what type of vaccine you had previously. For example, type "Pfizer #1, age 5" or "Moderna." (The CDC recommends that your third dose or booster dose be the same type of vaccine that you received for your first and second dose.)
