Care Connections Clinic at Mercy

Care Connections Clinic at Mercy is a unique clinic providing traditional primary care provider (PCP) for adults. In addition, this clinic serves a need for patients within the community by also providing short-term primary care needs for patients in between types of care, for example:

• Patients requiring follow-up care from an emergency visit.
• Patients needing to see a provider for additional follow-up after an inpatient stay.

Care Connections Clinic seeks to provide an option for patients who may not have a PCP or who are unable to see their PCP for immediate follow-up or pre-surgical care.

Care Connections Clinic at Mercy also accepts referrals for:

• Palliative Care, which may result in ongoing care from the clinic or consultations with recommendations and referral back to a PCP, as appropriate. Our providers have extensive experience in palliative care and can provide guidance and care planning to support patients suffering from pain or other symptoms of serious illness.
• Advanced Care Planning to support patients’ future decisions about medical care.
